Our Pledge

We at Hughes Animal Hospital agree to provide veterinary supervised boarding and care by our trained staff. Every effort will be made to provide a clean and caring environment for your pet(s) in your absence. We further agree to provide the following care twice daily: 1) fresh food and water, 2) outside exercise in our fenced-in walking area (dogs only), and 3) a clean and sanitized cage/run. We will also provide fresh bedding as needed.

Requirements for Boarding

Proof of vaccinations by a veterinarian are required for boarding.

Required vaccinations for dogs are:
Rabies, Bordetella, and Da2PPL (Distemper, Adenovirus, Hepatitis, Parvo, Parainfluenza and Leptospirosis)

Required vaccinations for cats are:
Rabies, Bordetella, and Felv-FVRCP (Leukemia, Calici, Rhinotracheitis, Chlamydia and Panleukopenia)

If your pet is not current on their vaccinations or if you are unable to provide proof of their vaccinations by a veterinarian, we can administer them upon arrival. Please select any vaccinations needed during their stay.

Medical Treatment

Medical Treatment Consent*

If your pet develops any medical condition during their stay, we reserve the right to treat as necessary to prevent the spread of illnesses. In the event that your pet experiences a life-threatening problem, we will make every attempt to contact you or your emergency contact listed on this form. If we are unable to make contact, we will proceed as the doctor deems necessary and you will be financially responsible for all treatments.

General Rules/Regulations/Information

Boarding animals will be admitted and discharged only during regular office hours. Our office hours are 8:00 a.m. –5:30 p.m. on Monday, Tuesday, Thursday and Friday, and 8:00 a.m. – 12:00 p.m. on Wednesday and Saturday. We are closed on Sunday.

If your pet is to be picked up by someone other than yourself, you must make these arrangements, including payment, at the time of drop off.

We are not staffed 24 hours a day; therefore, your pets will be unattended during the evening hours.

All animals must be free of fleas and ticks when admitted for boarding, or must be treated upon arrival, at the owner’s expense.

All precautions will be taken to prevent injury, escape or other life-threatening emergencies. The hospital and staff will not be held accountable for problems that develop, provided that reasonable care and precautionary measures have been taken.
